- Fill out the form below with your friends' contact information so we can reach out to them about perfecting their smiles with Smile Center Orthodontics or have them call 618-288-7000 to schedule a New Patient appointment. You must fill out the form below before your friend schedules their initial consultation.
- When your friend starts treatment with Smile Center Orthodontics, you will receive a Golden Ticket.
The more friends, who start treatment with Smile Center Orthodontics, the more Golden Tickets you will receive!
*In order to receive a Golden Ticket, referred patients must start treatment with Smile Center Orthodontics. Immediate family members (parents, children, siblings) are not eligible for Refer-A-Friend Program. Referred patients must provide your name when scheduling their initial consultation for you to receive a Golden Ticket.
